Adventist HealthCare seeks to hire an experienced Community Health Worker for our Care Navigation Department who will embrace our mission to extend God’s care through the ministry of physical, mental, and spiritual healing.
As a Community Health Worker, you will:
• Connect with individuals in the community to raise awareness of available behavioral health services and programs. Build trust and rapport to encourage participation in treatment.
• Assist patients in navigating the healthcare system by providing information about available services, scheduling appointments, and accompanying them to visits as needed.
• Identify and link patients to community resources, including support groups, social services, housing assistance, and educational programs to support their overall well-being.
• Provide education on behavioral health issues, treatment options, and coping strategies to patients and their families. Advocate for patients’ needs within the healthcare system and community.
• Regularly check in with patients to monitor progress, address challenges, and adjust care plans as necessary. Provide ongoing encouragement and support throughout their recovery journey.
• Maintain accurate records of patient interactions, referrals, and resource connections in compliance with facility policies and regulatory requirements.
• Work closely with behavioral health providers, case managers, and other healthcare professionals to ensure a coordinated approach to patient care.
• Respond effectively to crisis situations, providing support and stabilization for patients in distress.
Qualifications include:
• Bachelor’s degree in social work, psychology, nursing, or a related field required.
• Previous experience in a community health or behavioral health setting is highly desirable. Certified Community Health Worker preferred
• Licensed driver and able to operate a motor vehicle as well as maintain automobile insurance
• Active American Heart Association Basic Life Support (BLS) certification required
Work Schedule:
Work Schedule is Monday through Friday; 8 am-4:30 pm.
Pay Range:
$50,778.00 - $71,089.20If the salary range is listed as $0 or if the position is Per Diem (with a fixed rate), salary discussions will take place during the screening process.
