The Centralized Oncology Triage Advanced Practice Provider is a nurse practitioner or certified physician assistant who evaluates and determines medical appropriateness of oncology patients requiring unscheduled hospital admission to the University of Washington-Montlake (UW-ML) or University of Washington-Northwest (UW-NW) campuses. The Centralized Oncology Triage APP:

  • Medically triage oncology patients needing admission or transfer from an outside hospital to one of two inpatient oncology sites including UW-ML and UW-NWH
  • Assess clinical acuity and appropriateness via discussion with referring providers and chart review.
  • Monitor census for multiple inpatient oncology services and bed availability of inpatient oncology units.
  • Appropriately route patients to specific inpatient oncology services and units at multiple sites.
  • All clinical responsibilities are performed within the scope of practice and institutional privileges.

 

The Centralized Oncology Triage Advanced Practice Provider will also provide evaluation and medical management to inpatient oncology patients at the University of Washington Medical Center Montlake campus (UWMC-ML).

  • Provides care to patients who are admitted to the hospital for chemotherapy, management of toxicities of therapy, underlying disease, and concurrent illness.
  • Assess and manage medical needs of the inpatient oncology patients, addresses treatment toxicities, performs procedures and consults with professional peers as part of an interdisciplinary team working collaboratively with the treatment team to coordinate care of their patients.
  • All clinical responsibilities are performed within the scope of practice, institutional privileges and under the supervision of an attending physician.

Responsibilities

Centralized Oncology Triage:

  • Works independently and collaboratively to triage oncology patients requiring admission or transfer from an outside hospital, working working 12 hour rotating day shifts to cover 7 days per week.
  • Gathers pertinent clinical information via discussion with other providers and chart review to determine appropriate admission plan.
  •  to obtain hospital beds for oncology patients needing admission.
  • Coordinates patient admission with admitting services including UW-NW medical oncology inpatient service, oncology resident service, hematology-oncology Platinum service, hybrid service and medical oncology service at UW-ML.
  • Documents patient care data in accordance with Fred Hutchinson Cancer Center and University of Washington Physicians policies.
  • Other duties as assigned.

 

Inpatient Oncology:

  • Works as part of an interdisciplinary team on the inpatient units at UWMC-ML, working 12 hour rotating day shifts to cover 7 days per week.
  • Performs complex and problem-focused H&Ps; rounds daily with team and attending physician.
  • Evaluates and manages all aspects of care for medical oncology and hematology patients, with the possibility to include care for cellular and noncellular immunotherapy and bone marrow transplant patients depending on prior clinical experience.
