The Role
Under general direction of the Program Coordinator, has the primary responsibility for implementing the Family and Recovery Services for the Harlem Youth and Family Service Project. Communicates and coordinates with Community Partners, oversee the services provided by the Family Specialist, Therapist and Peer. Attends case conferences and provides updates on service planning for cases under the Administration for Children’s Services. Recruits through outreach and enrolls youth. Develops and implements individualized Family Partnership agreements with parents. Acts as community liaison for the agency.
Responsibilities
What You Will Do
- Organizes, facilitates and conducts meetings and training sessions for parents and caregivers.
- Oversees 1 home visit to each enrolled family.
- Ensures that Motivational Interviewing is being used and documented correctly.
- Trains program staff on case management and engagement through observation.
- Assists parents in developing individualized short-and long-term goals and in accomplishing them.
- Conducts assessments and individual session, using evidence base techniques.
- Ensures that CRAFT is used within the program staff.
- Reports all services conducted weekly to the Program Coordinator
- Maintains organized records of activities related to the Harlem Youth and Family Project.
- Coordinates efforts to encourage community members, caregivers and parents to seek out assistance for their identified family member
- Attends all scheduled management meetings within the program.
- Identifies community agencies and resources which provide services to migrant and seasonal families, establishing and maintaining cooperative relationships with them.
Qualifications
Who You Will Be
- Licensed Social Worker, with experience working with Families.
- Fluent in written and spoken Spanish and English required.
- Familiarity with child development or early childhood education.
- Knowledge of community resources and the social services system.
- Experience organizing, implementing, and managing multiple responsibilities under pressure.
- Experience in the use of computer data base and/or word.
- Ability to manage time efficiently.
- Ability to participate in or facilitate case management.
