About the role

The Advanced Practitioner /Clinical Program Director for Wound and Ostomy care is responsible for the development of a comprehensive Wound, Skin and Ostomy care program and insures the delivery of prevention and care in accordance with the philosophy, standards of practice, objectives and policies of the Department of Patient Care Services.  The Director provides advanced clinical leadership, strategic oversight and operational direction for the program within the surgical clinical practice. The Director is responsible for diagnosis and treatment of common problems related to the treatment, management and rehabilitation of patients with ostomies, wounds and/or continence problems as well as patient and family education and counseling and education of nursing staff. S/he is accountable for the budget planning and resources to enhance the quality of patient care, personnel and resource management, and in collaboration with senior nursing leadership, the annual goals, objectives and fiscal targets. The Director serves as an expert clinician, mentor and administrator, enhancing the patient and family experience, improving clinical outcomes and fostering interdisciplinary collaboration.

Patient Experience 

 

  1. Serve as the lead advanced practice provider for wound and ostomy management in the surgical clinical practice.
  2. Deliver expert consultation and direct patient care including assessment diagnosis and management of complex wounds ostomy and continence needs.
  3. Develop and implement individualized treatment plans that reflect patient and family goals
  4. Assure continuity of care and optimal outcomes for patients and families.
  5. Follows up on all patient concerns in a timely manner, provides service recovery as needed and escalates patient concerns as appropriate.
  6. Demonstrates caring, respect, compassion, empathy and active listening through dialogue, body language and actions.
  7. Addresses issues of communication, emotional support, and the complexities of managing wound, skin and ostomy conditions to prevent patients feeling overwhelmed by their conditions, and ineffective care can lead to feelings of neglect or frustration. 
  8. Promote an excellent patient experience by ensuring compassionate communication, timely interventions, and shared decision making.
  9. Advocate for patient and family engagement in self-care management, emphasizing dignity autonomy and cultural sensitivity.

 

 

Essential Duties and Responsibilities

 

  1. Develop, revise and evaluate standards of care    
  2. Assist in the selection of supplies, technology and equipment    
  3. Assure appropriate product utility while maintaining optimal patient safety and outcomes; identifies rental situations when appropriate                      
  4. Develop, provide and evaluate staff education programs, ie. Orientation, in services and continuing education              
  5. Conducts health assessment interview focused on wound, ostomy, continence related care       
  6. Performs focused physical assessment as appropriate.
  7. Orders and interprets diagnostic tests.      
  8. Develops plan of care based on the diagnosis and individual patient needs.        
  9. Prescribes medications and treatments as appropriate.    
  10. Works collaboratively with the health care team and makes referrals to other disciplines as indicated. 
  11. Records, reports and interprets the client’s responses to the plan of care.           
  12. Teaches, counsels, and assists individuals and families to assume responsibility for prevention of complications as they relate to patients with ostomies, wounds, or continence problems            
  13. Provides individualized fitting for appropriate medical equipment for in-hospital and outpatient ostomy patients       
  14. Maintain a minimum of 4-6 hours per week (scaling up to 8 hours based on patient demand) in direct, outpatient care within the surgical clinical practice, including but not limited to outpatient initial and follow up visits.
  15. Influence reductions in readmissions and length of stay through proactive outpatient management.
  16. Participate in surgical case planning and postoperative follow up for ostomy patients
  17. Ensure availability for advanced consults and staff support for both inpatients and out-patients on complex wound and ostomy cases.
