Job Description Summary:

***This is a full-time, day shift position at Pickerington Methodist Hospital. This role is a combination of two foci - 50% Antimicrobial Stewardship and 50% Clinical/Operations. Shift hours at 7:30am-3:30pm (evenings and nights as needed for staff coverage). Every 4th weekend required. JOB SUMMARY

This position is responsible for dispensing and compounding medication, counseling patients on medication and health subjects, interpreting prescriptions, managing patient drug therapy for complications, and performing drug utilization. This position collaborates with physicians, nurses, and other health-care practitioners to provide safe, high quality, and cost-effective drug therapy. Pharmacists will be required to serve as preceptors for pharmacy students and pharmacy residents in their area of training and expertise.

ANTIMICROBIAL STEWARDSHIP POSITION DESCRIPTION – INPATIENT (50%)
1. Leads antimicrobial stewardship strategy for accountable site(s) in collaboration with system antimicrobial stewardship team.
2. Develops, enforces and maintains policies and procedures that promote and enhance patient outcomes through safe, effective, cost-efficient and appropriate medication therapy use for patients receiving antimicrobials.
3. Coordinates programs designed to minimize medication errors, adverse drug reactions and medication misuse with regards to medication preparations in infectious disease through improved reporting, analysis and follow-up.
4. Coordinates and maintains the antimicrobial stewardship program, including:
a. Overseeing utilization audits of antimicrobial therapy
b. Assisting in development and maintenance of an antimicrobial formulary approved by the P&T Committee.
c. Working with the infectious disease service and hospital medical staff to evaluate appropriate use of restricted antimicrobials
d. Developing guidelines, policies, electronic health management solutions and other monitoring and intervention strategies for antimicrobial use and streamlining prophylactic, empiric, and definitive antimicrobial therapy
e. Reviewing data and metrics on antimicrobial use
f. Reporting data and findings to the Infectious Disease Clinical Guidance Council, and Pharmacy & Therapeutics Committees.
g. Coordinating, facilitating and promoting education, training, competency assessment and performance improvement of pharmacists, pharmacy residents and other healthcare professionals in the appropriate use of antimicrobials
5. Serves as pharmacy consultant to Infectious Disease consult service
6. Precepts pharmacy residents and pharmacy school students in Infectious Disease rotations.
7. Responsible for ensuring compliance with evidence-based recommendations and regulatory standards related to safe and appropriate antimicrobial use.


MINIMUM QUALIFICATIONS AND SPECIALIZED KNOWLEDGE

• Bachelor of Science or Doctor of Pharmacy degree from an accredited institution (Required)
• Current Ohio pharmacist license (Required)
• Basic Life Support Certification—current or obtained during the orientation
period (unless greater than 50% of scheduled work hours, as determined by
the manager, are completed off-site or not in a patient care setting)
• ASHP accredited residency or equivalent pharmacy practice experience
• Pharmacy Board Certifications or other certifications
• Advanced-level degree in healthcare field
• Collaborative practice experience
• Advanced Cardiac Life Support Certification (may be required to obtain as
determined by manager)
• Pharmacokinetic drug dosing experience
• Drug therapy management
