The Role:
- High-level clinical nursing assessment skills
- Involve patients/clients and their families/significant others in identifying achievable goals, focused on re-ablement and maintaining independence in the community setting

As RN TCPC you will provide expert clinical advice and clinical leadership within the transition care setting, supporting the effective assessment, case management, care coordination and implementation of planned, goal orientated transition care to older people in the community and/or residential aged care setting to enable long term care needs to be met in the most appropriate environment.

Details of appointment:
Permanent part time, day worker position, working up to 30 hours per fortnight, as soon as we can onboard you.
*notwithstanding hours to be negotiated with the successful applicant.

Salary: $75,902 to $102,908 pro rata per annum. Our Employer 11.5% superannuation contribution is on top of this amount.
- Salary range is in accordance with Nurses and Midwives (Tasmanian State Service) Agreement 2023
Allowances:
You’ll have access to salary package a range of benefits including living expenses up to $9,010 + $2,650 in meal entertainment each FBT year + other benefits if eligible.
- Access to the Grade 4 salary range $96,558 - $102,908 is subject to successful application for progression to Grade 4
- Post Graduate Allowance of up to 10% based on qualification type
