RN Registered Nurse Case Manager | Days -Local and Travel Accepted!

Are you an experienced Case Management RN with a passion for coordinating patient care and successful discharge planning? This travel opportunity offers the chance to work alongside an interdisciplinary team in a fast-paced acute care setting while making a meaningful impact on patient outcomes.

Position Details:

• Position: Registered Nurse (RN) – Case Management

• Location: Bronx, NY

• Assignment Length: 13 Weeks

• Shift: 8-Hour Days, Monday–Friday

• Schedule: 8:00 AM – 4:30 PM

• Guaranteed Hours: 37.5 hours per week

Requirements:

• Active NY RN License

• BSN required

• Minimum 1 year of inpatient Case Management experience

• Experience with acute care discharge planning

• BLS Certification

• PRI (Patient Review Instrument) Certification

• SCREEN Training Certification

• Ability to work independently while collaborating with an interdisciplinary team

• Strong critical thinking, communication, and time management skills

• Experience with CarePort, MCG, InterQual, or Utilization Review is preferred

• Prior critical care experience is preferred
