About the role
Essential Duties:
- Assesses participant’s physical and functional needs, along with psychosocial status and needs, utilizing professional knowledge, skills of observations and interviewing skills.
- Documents change in the participant’s condition noted in clinic and inform appropriate care members.
- Regularly informs the interdisciplinary team of the medical, functional, and psychosocial condition of each participant.
- Assumes responsibility for continued professional growth, such as in-service programs.
- Responsible for day to day operation and activities of the clinic
- Responsible to ensure that clinical procedures requested are performed in the appropriate time frame
- Interface with transportation companies to ensure residents will be transported to appointments.
- Deal effectively, professionally and tactfully with residents and families. ( add participant instead of resident
- Perform TB test or QuantiFERON blood test for employees when indicated
- Administer flu and COVID to employees when needed.
- Administer influenza, pneumonia, hepatitis and PPD to participants
- Demonstrates the ability to recognize priorities in organization of workflow. Able to perform duties independently with minimal need for direct supervision.
- Responsible for tracking and documenting PHS for participants
- Responsible for daily and weekly logs in clinic.
- Responsible for ensuring any item that is expiring is reordered
- Notify nurse manager or Program Director when equipment not working properly.
- Ensure steady follow of clinic
- Coordinate with scheduler when a ppt is due to be seen in clinic.
- Perform other duties as assigned.
- Current NY State Registered Nurse License
- Associate Degree
- BSN Preferred
