The Role:
- Initiate and maintain internal and external educational programs to facilitate the provision of care for older people in the most appropriate setting in collaboration with the NUM and the multidisciplinary team
- Provide an advanced level of professional clinical expertise and management to ensure planned, goal-oriented transition care to older people
- Consult with the health care team and contribute to recommendations in relation to planning, implementing, and evaluating clients care to ensure best practice
- Champion a culture that upholds the National Principles for Child Safe Organisations and ensures the safety of all vulnerable people.
- Advocate for the safety, wellbeing, and empowerment of the frail elderly patients in our care

As CNC TCPC you will provide expert clinical advice and clinical leadership within the transition care setting, enabling the effective assessment, case management, care coordination and implementation of planned, goal orientated transition care to older people in the community and/or residential aged care setting to enable long term care needs to be met in the most appropriate environment
Details of appointment:
Permanent part time, day worker position, working 20 hours per fortnight, commencing as soon as we can onboard you.
*notwithstanding hours to be negotiated with the successful applicant.
Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.
Salary: $111,506 to $118,336 pro rata per annum. Our Employer 11.5% superannuation contribution is on top of this amount.
- Salary range is in accordance with Nurses and Midwives (Tasmanian State Service) Agreement 2023
