Position Summary
The Quality Analyst 3 provides assistance with gathering, monitoring, reporting and evaluating clinical quality, in accordance with Heartland Health Services’ (HHS) mission and strategic goals, federal and state laws and regulations and accreditation standards.
Essential Functions
- Analyzes, researches, and reports on quality improvement activities in order to meet established guidelines set by HHS and other regulatory and accreditation bodies.
- Completes quality audits on assessments, screens, and other clinical documentation to improve outcomes, processes and to ensure adherence to policy, procedure and applicable state and federal regulations.
- Organizes and performs ad hoc quality focus studies on assessments, screenings, and other clinical documentation to identify trends, develop recommendations, and guide quality improvement activities.
- Supports or leads projects intended to improve clinical quality/operational measures, to support QAUR committee recommendations, and/or to support the organization/s QI/QA plan.
- Collects and analyzes clinical data upon requests, monitors over time, and recommends improvement strategies.
- Analyzes and leads initiatives related to optimizing the EMR as it relates to reduction of errors, efficiency, and strategies to best comply with UDS reporting. Participates in monthly Epic meetings.
- Oversees the quality initiative workflow with patients and third-party payers.
- Assists with education and training for staff on quality improvement activities related to clinic organizational objectives, compliance, and individual job functions, i.e. OSHA, HIPAA, etc.
- In collaboration with the VP of Quality and Compliance and Quality Analyst 1, develops tools and resources for new and existing quality improvement projects and initiatives, including various presentations, training plans, forms, spreadsheets, informational handouts, etc.
- In collaboration with the VP of Quality and Compliance, assists with maintaining, updating and communicating quality policies and procedures throughout the company.
- Works in consultation with clinical teams, direct clinical support staff, and indirect clinical support staff to support policies and procedures that maximize patient-centered communication and services.
- Maintains current knowledge of guidelines and requirements imposed on an FQHC.
- Establishes and maintains professional working relationships with all members of the HHS organization.
- Supports clinic compliance with all applicable federal, state, local, and HHS rules, regulations, protocols, and procedures governing the clinical provision of medical services as well as those relating to, but not limited to, workplace safety, public health, and confidentiality.
- Supports and is involved in HHS’s continuous quality improvement efforts designed to improve patient outcomes.
- Works in consultation with clinical teams, direct clinical support staff, and indirect clinical support staff to develop and implement policies and procedures that maximize patient-centered communication and services.
- Maintains and assures confidentiality of patient information in accordance with HHS’s policies.
- Reports building/equipment problems through the appropriate channels.
- Performs any clerical duty or department related task as assigned by supervisor in a continuously changing medical practice.
- Attends all staff meetings, department meetings, and any other meetings as required.
Requirements
- High School Diploma or equivalent required. Bachelor’s degree preferred, RN or LPN highly recommended.
- Minimum of 3 years of clinical experience
- Extensive experience with the use of an Electronic Medical Record, EPIC preferred.
- Effective use of computer programs, including Word, Excel, Outlook, PowerPoint and database management
- Solid understanding of how to efficiently and accurately audit and analyze complex information
- Knowledge of quality improvement principles, processes and practices
- Possesses a high degree of integrity and strong work ethic
- High level of initiative, organizational and problem-solving skills
- Ability to interact professionally will a variety of people, including providers
- Ability to deliver effective training
- Ability to handle job related matters in a professional, diplomatic, and confidential manner
- Ability to travel to all locations and to meetings outside the service area; core hours with flex time required
