About the role

Job Summary:

Responsible for providing medical support for the individuals, advocate for additional medical supports when appropriate, and provide staff and individual training in health related areas. Provide medical oversight to CSNH Managers and Staff.


Supervisory Responsibilities:

None.


Essential Duties/Responsibilities:

Identify health related needs, monitor medical status and work with staff, physician’s and allied health professionals to formulate, coordinate and implement medical treatment plans and/or wellness plans for all consumers.

Administer medications and treatments as prescribed, ensuring adherence to BDS, Medicaid and other NH state regulations.

Provide individual and/or group training in relevant areas (e.g., health hygiene, safety, proper diet, medical equipment, medication administration, etc.).

Coordinate procurement of medical supplies needed for treatments and supports as applicable

Provide direct support to individuals when indicated

Provide interface to CPS / day programs as needed to ensure continuity of support

Complete written reports, assessments and other documents as required by the organization for funding and licensing.


Documentation must be completed in a clear, concise and professional manner. This includes but is not limited to; nursing communication, ISA and HRST documentation.

Advise the CSNH Director, Program Coordinators, House Managers and staff of any medical problems that may hinder an individual’s progress or that impact their involvement in care.

Ensure that needed medications are dispensed by staff.

Perform duties working within a team approach.


Required Skills/Abilities

Flexible schedule – Provide 24/7 on call support to CSNH Management team for the care of individuals.

Strong verbal and written communication skills.

Excellent interpersonal and customer service skills.


Required Education and Experience:

Nurse Trainer Certificate by the State of New Hampshire

ASN – Associate Nursing Degree

Registered Nurse by the State of New Hampshire

Two years of licensed nursing experience

Knowledge of rehabilitative care, ISA development and behavior management for adults with disabilities.


Preferred Education and Experience:

BSN – Bachelors Nursing Degree


Travel requirements:

Heavy Travel - example once or more a week
