Brookdale Lake Shore Drive is hiring a Director of Resident Transition Services
A Senior Living community specializing in Independent and Assisted Living
Requirements:
- 1-2 years of Leadership / Management experience
- RN, LPN, or Social Worker Preferred
- Experience / knowledge of the Elderly / Geriatric population
- Understanding of how to connect seniors to services they need/want
- Case Management / SW experience preferred
- This is an incentive based position, which may include bonuses, incentive or commission plans.

Responsibilities
Assists residents in identifying appropriate resources in order to maintain their highest level of independence and satisfaction to include problem solving for health, social, and emotional needs and coordinating support services. May be required to supervise other staff positions.
