Family Support Coordinator

WHAT YOU WILL DO:

The Family Support Coordinator is a non-exempt position responsible for intake coordination and the delivery of case management services to at-risk families of child abuse and neglect including parenting skills training, home safety and life skills, job placement, housing, school tutoring, and other support services to reduce the likelihood of any child abuse or neglect incidents. Duties and Responsibilities:

• Conduct home visits with parents, caretakers, at-risk children, their families, and extended support networks
• Help parents improve their relationships with their children and provide better care, nutrition, hygiene, discipline, protection, instruction, and supervision
• Help parents provide a better household environment for their children by teaching household maintenance, budgeting, and purchasing
• Provide information, referrals, and assistance in locating appropriate childcare
• Assist parents in meeting the special physical, mental, or emotional needs of their children, and help parents deal with their own special physical, mental, or emotional needs that interfere with their ability to care for their children and manage their households
• Assist families in discovering and gaining access to community resources that can assist in meeting their needs
• As a part of the Family Reunification Team, will maintain at least weekly telephone or personal contact with the Case Management Case Manager
• Prepare documentation and present assigned case during internal staffing as necessary
• Respond to emergencies and provide crisis intervention, and on-call responsibilities
• Perform all other duties as assigned
