Description
The Nurse-Family Partnership (NFP) Nurse Home Visitor is responsible for providing comprehensive community health nursing services to women and their families eligible for the NFP program. Nurse Home Visitors (NHV) are pivotal in transforming the lives of vulnerable babies, mothers, and families by implementing and maintaining the fidelity of the NFP model. The NHV is responsible for maintaining the highest standards in clinical nursing practice, adherence to the polices, procedures, guidelines, as well as the standards of NFP and of the implementing agency. Our goal is that all children are healthy, families thrive, communities prosper, and the cycle of poverty is broken.
Essential Duties and Responsibilities:
- Provides home visits, in-home and/or virtual, to women and their families eligible for the NFP Program.
- Adheres to nursing process and the NFP model of home visitation.
- Develops and maintains community relationships to promote public awareness and to support client referrals.
- Carries a caseload of 25-30 clients, when working as a full-time NHV.
Requirements
Education –
- Bachelor of Science in Nursing preferred or equivalent combination of education and experience.
- Current RN license (in good standing) required
Experience –
- Home visiting experience preferred.
- Two years recent experience in maternal/child health, public health, home visiting or mental/behavioral nursing preferred
- Bilingual in Spanish/English preferred, not required.
Regulatory –
- Minimum 21 years of age.
- A clean 39-month Motor Vehicle Report with no more than 2 moving violations.
- Valid, unrestricted Arizona driver’s license and the willingness to use personal vehicle in the course of employment. Must meet minimum requirements of agency insurance carrier.
- Must possess a valid AZ DPS Level 1 fingerprint clearance card.
- CPR and First Aid certification (Employer provides).
- Initial current negative TB test result (Employer provides).
- Proof of COVID-19 vaccination required and willingness to obtain future boosters if required by employer. Accommodation available.
Skills:
- Written and Verbal Communication- NHVs need to communicate clearly, effectively, with confidentiality, openness and professionally with their families, colleagues and stakeholders.
- Time Management- NHVs who can flexibly schedule and reschedule a day’s activities based on multiple factors.
- Listening- interactions between NHVs and clients depend on the NHVs ability to listen attentively and accurately to the client’s current experience.
- Ability to impose structure- in between and during home visits, NHVs must be able to create structure in moments of confusion, disorganization or crisis.
- Professional Development- NHVs will assess own learning needs, develops strategies to meet them, and will document and record progress toward goals. Will be motivated to utilize computer for long-distance learning. NHVs will stay informed of current health care developments to provide safe, evidence-based, and quality nursing services.
Physical requirements:
- Must be able to utilize Personal Protective Equipment (PPE), such as face mask, gown, goggles and gloves, for any amount of time necessary to provide a physical barrier to protect the employee and others.
- Must be able to remain in a stationary position.
- Must be able to operate a computer, phone and other office equipment.
- Must be able to stoop, squat, reach, stretch, ascend and descend stairs.
- May work in an office setting.
- Must be able to work a flexible schedule including some evenings and weekends
