Job Description Summary
This position provides advanced population-based health promotion and health education coordination and outreach with the ability to work with minimal supervision and make independent recommendations. Position collaborates with the Public Health Outreach Nurses on using evidence-based strategies to promote health and well-being and uses independent judgement. Position will be required to drive the Mobile Medical Unit to Outreach events in Weld County. This includes self-directed planning, developing, implementing, and facilitating community-based strategies related to health promotion. Must be self-directed, have excellent organizational and multi-tasking skills, and have advanced group facilitation skills. Must demonstrate ability to speak effectively and professionally to a variety of audiences. Position requires advanced systems thinking and problem-solving skills, including knowledge and skills using data analytics. Also provides communications and other public health related support during emergencies or as needed.The ideal candidate for this position is a self-directed professional with strong public health education, outreach, and program coordination skills. They excel in communication, engaging diverse communities and building strong partnerships with stakeholders. They thrive both independently and in team settings and will work closely with our Public Health Outreach Nurses. They will be required to drive our Mobile Medical Unit to outreach events in our community. Preferred qualifications include bilingual proficiency in Spanish.

Job Description
Program organization and coordination - 60%:
- Selects or develops advanced program components (goals, objectives, strategies, interventions, and activities) in a structured format to address health education and public health objectives using evidence-based strategies. Initiatives supported include the Community Health Assessment (CHA), the Community Health Improvement Plan (CHIP), and/or the Public Health Accreditation Board (PHAB) requirements or other documents.
- Promotes and expands advanced strategies for adults, youth, or both, and community engagement with a variety of population groups.
- Self-directed and has excellent organizational and time management skills.
- Collaborates with various partners and community groups to ensure the success of various grant workplans and deliverables; forms workgroups as needed to sustain activities and meet program goals. Is an active member in outside committees, associations, or other program specific groups.
- Conducts professional public speaking to various groups and audiences; identifies problems and develops advanced solutions and takes a self-directed approach to solving the problems; develops and maintains an advanced evaluation plan to ensure effectiveness of health education and public health problem solving; makes recommendations for future programming; completes program reports as required.
- Monitors and coordinates budget expenditures for the program, issues purchase orders and requests for program supplies and expenses. Identifies and accesses funding for programs by writing grant proposals.
- Performs as a member of the health promotion team to diagnose and track community health issues and helps to solve them through the selection and use of sound population-based strategies. Assists municipal governments, schools, or community partners with the development of health promotion or health education efforts.
- Displays a high level of initiative, effort, and adaptability. Works with minimal supervision and demonstrates responsible behavior and attention to detail.
- Drives Mobile Medical Unit to community outreach events.
Communication and engagement - 35%:
- Ability to be the lead author for writing correspondence and reports to funding agencies, community leaders, journals or for other health professional organizations.
- Identifies complex problems and reviews related information to develop and evaluate options and implement solutions, using independent judgement and recommendations.
- Uses exceptional customer service skills to respond to community needs, enhance public relations, coordinate activities, and represent the Weld County Department of Public Health and Environment to promote and maintain a positive public health image and foster pride and professionalism in the workplace and community.
