We hope you will apply to become part of our dedicated, fast-moving team of superstars! ZoomCare is seeking a Medical Director of Primary Care to join our team! The Medical Director of Primary Care is responsible for leading clinical efforts across ZoomCare’s primary care operations, driving performance in key clinical quality metrics, Hierarchical Condition Category (HCC) coding recapture, and certification as a Patient-Centered Medical Home (PCMH). This leadership role requires a dynamic individual who thrives in a collaborative environment and is passionate about improving patient outcomes, optimizing clinical processes, and supporting primary care physicians and teams. SCHEDULE AND TRAINING
- High availability day and night for on-the-spot problem solving.
- Direct clinical care of approximately up to two full days (8 hours/day) per week where applicable. Approximately one day of clinical support per week (17 hour shift) and approximately one day of protected administrative time per week (8 hours).
- Clinic positions require you to attend 2-3 weeks of training. The first week will be at your primary location and partially remote, the second and third weeks include traveling to multiple clinics in the area.
- Depending on the position and associated requirements, there may be additional mandatory training requirements that are outside of your scheduled shift.

- Lead efforts to achieve and maintain optimal clinical quality outcomes across primary care, with a focus on preventive care, chronic disease management, and population health.
- Develop and implement strategies to improve key quality metrics (e.g., HEDIS, STAR Ratings, preventive screenings, chronic care management), and monitor/report clinical quality data to provide actionable insights to teams and leadership.
- Act as a champion for continuous quality improvement, fostering a culture of learning, innovation, and clinical excellence.
- Stay current with healthcare trends, regulatory changes, and industry best practices to implement top-tier clinical approaches.
- Oversee HCC coding recapture training and implementation to ensure accurate documentation that reflects patient complexity.
- Train and support clinical teams on proper coding techniques to optimize risk adjustment and reimbursement.
- Analyze coding trends, identify areas for improvement, and collaborate with clinical and administrative teams to ensure accuracy and compliance.
- Co-lead the initiative to achieve and maintain PCMH certification in partnership with operations and administrative leadership.
- Develop and implement strategies to enhance care coordination, patient engagement, and care team collaboration in line with PCMH principles.
- Ensure clinical workflows are optimized for patient-centered care, emphasizing access, continuity, and comprehensiveness.
- Provide thought leadership in transforming ZoomCare’s primary care services through new technologies, workflows, and care models.
- Work closely with the Chief Medical Officer, Clinical Operations, and other leaders to develop and implement long-term strategies for clinical excellence.
- Mentor and support primary care providers with coaching on clinical decision-making, patient management, and quality initiatives.
- Lead initiatives to streamline operations, reduce inefficiencies, and improve patient satisfaction.
- Serve as the Practice Lead for Family Medicine.
- Share on-site supervision duties for new providers during their first 3 months in collaboration with practice leads.
- Represent ZoomCare’s clinical leadership team in meetings, initiatives, and external engagements.
- Participate in meetings with the Chief Medical Officer and other clinical leaders as agreed upon.
- Contribute to the Primary Care Quality Report and oversee relevant quality measures.
