Responsibilities

The Population Health Care Coordination Specialist, under the direction of the Manager of Population Health and their supervisor, has primary responsibility for care management activities for a specific patient panel.  This will include identifying the high acuity patient population and developing, conducting, and monitoring care management processes. 

Qualifications

REQUIRED

Bachelor of Science Degree in Nursing and a minimum of 3 years of experience in a clinical setting OR Associates Degree in Nursing with a Bachelor of Science Degree in a health related field with a minimum of 5 years’ experience in a clinical setting

Proficient computer skills, including Microsoft Office (specifically Word and Excel)

Excellent oral and written communication skills.

Good organizational, analytical, and problem-solving skills.

Ability to function effectively with a wide variety of disciplines, including physicians and nurses.

A team player that can follow a system and protocol to achieve a common goal.

Confidence to communicate and collaborate with other community health care organizations and personnel.

Able to participate as part of a team to accomplish an assigned project.

PREFERRED

Previous experience in a Primary Care Setting.

Previous experience in case management, care coordination, or patient navigation.

Previous population health experience.

Work Hours:

8am - 4:30pm
