Behavioral Health Clinician

Overview

 Behavioral Health Clinician integrated within a primary health care clinic to function as a member of an interdisciplinary team to provide screening, brief evidenced based treatments, and care coordination for consumers of healthcare and their families

 

Responsibilities

 

 Interpersonal Communication- The ability to establish rapport quickly and to communicate effectively with patients, family members and providers.--------------------------------------------------Intervention- The ability to provide a range of brief, solution-focused treatment for prevention, health promotion, and recovery services, as well as develop longer-term treatment plans for patient when indicated.--------------------------------------------------Screening and Assessments- The ability to conduct brief, evidence-based and developmentally appropriate screening and to conduct or arrange for more detailed assessments when indicated.--------------------------------------------------Care Coordination- The ability to create and implement integrated care plans, ensuring access to services, helping to prevent over utilization of resources/readmissions--------------------------------------------------Evidence-based practice & Quality Improvement- The ability to identify and implement evidence-based practices, assessing treatment fidelity, measuring patient satisfaction and healthcare outcomes, and collaborating with other team members--------------------------------------------------Cultural Competence- Identifying and addressing disparities in healthcare access and quality, adapting services to language preferences and cultural norms--------------------------------------------------Collaboration- The ability to function effectively as a member of an inter-disciplinary team that includes behavioral health, providers, patients, their families and other systems involved--------------------------------------------------

Qualifications

 

EDUCATION:Masters, Required

 

EXPERIENCE:Preferred: One-two years clinical experience working in the pediatric population; An equivalent combination of education and experience may be considered. If working with children, preferred experience working with children & adolescents.
