ServiceSource is seeking a Case Manager to support a new program serving Fairfax County Redevelopment and Housing Authority (FCRHA) households as they work toward housing stability, increased financial independence, and long-term self-sufficiency.

In this role, you will:
• Conduct comprehensive assessments and develop individualized service plans
• Provide ongoing participant-centered case management and support
• Coordinate housing stabilization efforts and connect families to critical resources
• Facilitate referrals and collaborate with community partners and service providers
• Help individuals and families navigate barriers related to housing, employment, benefits, healthcare, and other essential services

Case Managers play a critical role in the multi-year contracted program designed to help Fairfax County Redevelopment and Housing Authority (FCRHA) households stay stably housed while building the income, skills, supports, and confidence needed to move toward long-term self-sufficiency. As an integral part of the support team, the Case Manager works to move individuals toward self-sufficiency by linking individuals and families to essential community resources, coordinating referrals and applications, housing stability supports, and assistance in navigating barriers to employment. Using a wrap-around approach, the Case Manager develops care plans, continuously assesses changes in status and support needs, and updates care plans as necessary to ensure ongoing support.

Primary Duties

  • Complete comprehensive assessments and develop initial Individualized Training and Service Plans (ITSPs) based on participant strengths, immediate needs, family priorities, service intensity, and self-sufficiency goals.
  • Provide participant-centered case management through regular engagement, at least monthly contacts, and ongoing review of progress toward individualized goals.
  • Develop, implement, and update person-centered plans that reflect changing participant needs, employment goals, housing stability needs, and long-term self-sufficiency objectives.
  • Coordinate housing stabilization efforts and collaborate with FCRHA staff, housing navigators, employment specialists, TANF case managers, school counselors, community providers, and other partners to ensure wraparound support.
  • Facilitate referrals and warm handoffs to internal and external resources while maintaining accurate, timely documentation of services, contacts, referrals, participant progress, and outcomes for compliance and reporting.

Additional Responsibilities

  • Build and maintain long-term relationships with community partners, participants, their families, and other stakeholders (e.g., medical teams, paid and natural supports, legal guardians) to ensure access to up-to-date resources and support.
  • Regularly communicate and collaborate with FCRHA support staff.
  • Stay informed on current and emerging community resources, including programs and services related to healthcare, housing, education, employment, and other relevant supports.
  • Build and maintain partnerships with local organizations to ensure access to the most up-to-date information, enabling effective referrals and support for individuals and families.
