Your Specific Responsibilities:

The position is for a lead physician to manage health care for 30 of our more complex and high-risk patients.

This position will act as lead physician for a new "Intensive Case Management Program" and will include a separate clinic, collaborating closely with the other free clinics currently at GRACE.

We anticipate operating 4 hour in-person clinic one afternoon per week.

The lead physician will have administrative and nursing support in the clinic, and oversee continuing care provided by a full time nurse or paramedic working in the field to follow up, monitor and assist patients with their care plan.

 

Required Knowledge, Skills, and Abilities:

Using scientific methods to solve problems. Observing and evaluating the outcomes of a problem solution to identify lessons learned or redirect efforts. Knowing how to find information and identifying essential information. Using logic and analysis to identify the strengths and weaknesses of different approaches

Qualifications:

Minimum – Active Florida Medical License

be comfortable working in a low-resource setting, providing trauma-informed care for individuals facing significant medical and socioeconomic challenges.

Preferred – 2-3 years experience working with at risk population.
