Patient Coordinator

This is a remote position.

• Coordinates the patient’s and family care and services, including those of the team, social worker, home health aide and physician
  
• Responds necessary information to the Director of Nursing Monitors the re-certification / re-authorization process
  
• Ensures and documents appropriate use of the medications, durable medical equipment and supplies
  
• Educates patients, families and other caregivers Appropriately utilizes the skills and resources of contract personnel such as PT, OT, Podiatrist and Pharmacy
  
• Ensure services are being performed appropriately
  
• Participates in routine chart reviews Attends regularly scheduled in-services.

Requirements

• Experience in healthcare administration, case management, or related healthcare coordination preferred
  
• Strong organizational and time management skills with ability to handle multiple priorities
  
• Proficiency with data entry and healthcare software systems
  
• Excellent communication skills for effective patient and provider coordination
  
• Ability to work Pacific Standard Time hours consistently
  
• Exceptional attention to detail for clinical documentation accuracy
  
• Commitment to delivering top-level performance in a healthcare environment
