Position Purpose: Directly supervise clients while assisting psychiatric providers and nursing with client health history/assessment at psychiatric appointments. Provide continuity of program services in an ethical, legal, and moral manner within a safe and therapeutic environment, consistent with applicable regulatory and accreditation standards.
Qualifications/Basic Job Requirements:
• High School Diploma or GED and one year of addiction treatment experience or an equivalent combination of education, training, and experience
• Certified as a CNA, CMA, EMT or greater
• Cardiopulmonary-resuscitation (CPR) and First-Aid training complete within three months
• Experience utilizing behavioral management techniques
• Ability to lead discussion and educational presentations
• Valid Illinois driver's license and ability to meet agency insurance requirements
• Belief in the mission and vision of Rosecrance
Essential Responsibilities:
- Adhere to prescribed work hours set by leadership and fulfill additional hours as necessary to complete assigned responsibilities.
- Record and respond to calls, check records for compliance and insurance prior to scheduling appointments and determine appropriate platform to be used.
- Schedule client follow-up sessions promptly after their psychiatric appointments, as deemed suitable and necessary.
- Coordinate psychiatric appointments for clients in collaboration with direct care staff.
- Ensure that all psychiatric appointments are fully booked to achieve a 100% appointment fill rate.
- Maintain accurate and up to date logs for clients with approved scheduling and ensure notification is in the chart to allow for accurate and timely scheduling of follow-up psychiatry visits.
- Maintain accurate and up to date logs for clients for whom approval to Physician Services only has been deemed appropriate.
- Coordinate continuity of care across facilities by proactively obtaining hospital discharge documentation or other pertinent collateral information from external sources ahead of clients’ scheduled psychiatry appointments.
- Offer clerical assistance to medical leadership, including the entry of note templates for psychiatric providers.
- Follow up on unposted or draft notes for psychiatric providers, addressing the matter as necessary, or informing the provider if required.
- Provide backup coverage for medical staff within the medical suite.
- Conduct client health history assessments, assist with vital sign assessments, and provide reports to psychiatric providers prior to appointments.
- Transcribe medical orders.
- Provide follow up communication to clients with ongoing medical concerns.
- Monitor and document client’s response to medication and report any notable response immediately.
- Document pertinent information daily in the electronic health record based on observations and involvement with the clients as required.
- Act as an advocate for all clients and report all physical/psychological concerns to the medical and clinical staff.
- Contribute actively as an integral member of the clinical team, fostering collaboration and inclusivity by participating in all team meetings, discussions, and activities aimed at enhancing client care.
- Collaborate with medical records team to ensure chart compliance as appropriate.
- Understand and comply with all of the principles established by the Rosecrance Corporate Compliance Program and Code of Conduct.
- Deliver exceptional customer service consistently to every customer.
- Serve as a role model for other staff, clients and customers and demonstrate positive guest relations in representing Rosecrance.
- Assume any additional responsibilities as assigned by leadership.
Schedule: 8-hour shifts
- Shift: 9:00am - 7:30pm
- Monday - Wednesday, every other weekend
