Overview
Cadence Health is seeking an experienced Registered Nurse (RN) leader to lead RN and LPN teams responsible for triage and alert management within our Remote Patient Monitoring (RPM) programs. This role plays a critical part in ensuring timely, evidence-based interventions for patients managing hypertension, diabetes and congestive heart failure.
This position is ideal for a nursing leader who has managed teams in a virtual care or centralized triage environment and thrives in a fast-paced, technology-enabled care model.
WHAT YOU’LL DO:
- Lead and mentor RN and LPN teams supporting triage and alert management.
- Provide coaching, feedback, and development to drive clinical excellence.
- Oversee staffing, scheduling, and workload to ensure efficient, high-quality care.
- Foster a collaborative, patient-centered culture grounded in accountability and empathy.
- Reinforce adherence to clinical protocols and escalation standards.
- Monitor documentation quality and ensure audit readiness at the team level.
- Track and act on productivity, escalation, and performance metrics.
- Support onboarding and training of new clinical team members.
- Foster accountability, engagement, and team consistency
- Escalate systemic issues and risks to Care Delivery Operations leadership.
WHAT YOU’LL NEED:
- Active RN license in good standing (multi-state license preferred).
- 5+ years of clinical RN experience.
- 2+ years in a leadership or supervisory role within triage, care coordination, telehealth, or virtual care.
- Experience working in a startup or high-growth, fast-paced environment (health tech strongly preferred)
- Strong foundation in primary care and chronic disease management, especially HTN, DM, and CHF.
- Experience managing remote RN/LPN teams in high-volume environments.
- Proficiency with EHR/eRecord systems and data analysis tools for performance monitoring.
- Strong leadership, communication, and problem-solving skills.
- Skilled in nursing processes, documentation, and clinical decision-making
- Technical fluency; able to work seamlessly across platforms including Notion, Athena, EPIC, Zendesk, and G Suite.
The anticipated base salary range for this role is $90,000-$120,000 annual base salary, which is based on role scope, level, and location. In addition to base compensation, this role may be eligible for incentive compensation as part of the overall total rewards package. This position is remote and based in the United States. Actual compensation will be determined based on factors including experience, skills, internal equity, and applicable law.
