We are seeking a dedicated AOT Case Manager to join our community outreach team in Seattle. In this role, you’ll provide hands-on case management, recovery support, and resource coordination for clients in the Assisted Outpatient Treatment (AOT) program—meeting them in the community, wherever they are on their journey.
This is a hybrid position, and the schedule is Monday-Friday, 8:00 am to 4:30 pm. The pay range is $25.00 to $31.00 per hour, depending on experience.
Requirements
- Provide case management services to patients referred to or enrolled in the AOT program, following approved guidelines.
- Conduct outreach to engage patients and support their continued participation in treatment—especially those who are difficult to engage.
- Help patients access resources through care coordination and advocacy. This may include housing, transportation, basic needs, daily living support, financial planning, vocational services, and behavioral health care.
- Support patients in setting and progressing toward recovery goals. Accurately document goals, objectives, and interventions in the electronic medical record (EMR).
- Assist with de-escalation and provide appropriate interventions when needed.
- Foster a positive, empathetic, and non-judgmental environment for all patients.
- Use an agency vehicle to provide outreach and transport patients to approved services and resources in the community.
- Maintain strict patient confidentiality at all times.
- Document all services and interactions in the patient’s EMR promptly and accurately.
- Submit all required documentation to your supervisor or manager on time, in accordance with AOT contract requirements.
- Attend Involuntary Treatment Act (ITA) and AOT court hearings as assigned.
- Coordinate care with internal and external providers to help patients meet their treatment goals.
- Perform other duties as assigned.
MINIMUM REQUIRED TRAINING AND EXPERIENCE:
- Minimum high school diploma or GED and continuing education in related field.
- WA State Registered Agency Affiliated Counselor credentials required.
- Bachelor's degree in social services related field with internship (e.g., social work) OR at least 2 years of experience as a case manager.
- WA State Driver’s License and a clean driving record (driving record background check will be obtained).
- Extensive knowledge of community resources and ability to facilitate/coordinate access to those resources.
- Experience in working with individuals who are diagnosed with a substance use disorder and/or mental health.
- Experience in utilizing an electronic medical record system.
