About the role
LCSW (Clinical Care Coordinator) –Norwalk, Weston, New Canaan PC locations, Fairfield in Connecticut Nuvance Health is recruiting part-time Clinical License Social Worker (LCSW) at Norwalk and Weston Primary Care locations in Fairfield in Connecticut. The Clinical Care Coordinator will provide support to the Behavioral and Mental Healthcare department following a Monday-Friday office schedule of 8:30am-5pm in an outpatient setting. Furthermore, Western CT Medical Group Primary Care Offices serve Newtown, Brookfield, Ridgefield, Southbury and Danbury. The Clinical Care Coordinator will work five hours per week at each location, performing behavioral health assessments, referrals and recommendations for treatment and community services. Summary: Performs assessments and formulates treatment plan and or discharge/transition plans in collaboration with patient, provider, and possible family and multidisciplinary healthcare team. Provides ongoing monitoring and evaluation of plans to meet the individual needs of patients throughout the care continuum. Assists in resource management through facilitation of care and participation in program planning, data collection and analysis. Provides education to patients/families regarding coverage, care options and community resources. Qualifications:
- Master's degree in Social Work from an accredited school
- Minimum of 3 years’ experience
- Current CT Licensed Clinical Social Worker (LCSW) in NY & CT
