Registered Nurse Specialist 

 

This Registered Nurse Specialist is a nurse home visitor in the Pinellas Nurse-Family Partnership (NFP) responsible for providing comprehensive nursing services in the specialized area of maternal and child health, specifically with low income, first time mothers and parenting women and their family eligible for the NFP Program. The nurse home visitor is responsible to work independently maintaining the highest standards in clinical nursing practice and adherence to the NFP model, and to policies, procedures, NFP Visit-to-Visit Guidelines and standards of NFP and of the lead agency. The nurse home visitor reports to the NFP Nurse Supervisor.

 

Your Specific Responsibilities:

The NFP nurse home visitor carries a case load of up to 25 families. Home visiting services begin during pregnancy and extend to the child’s second birthday. The nurse arranges home visits and performs nursing interventions in the mother and child’s home environment or location of choice. Services are provided per the NFP schedule and in accordance with the NFP model and guidelines and the nursing process. Minimum of 8-10 successful home visits expected weekly at maximum capacity. Develops long-term therapeutic relationships and utilizes concepts of reflection and motivational interviewing during home visits. Assesses physical, emotional, social and environmental needs of women and their families as they relate to the NFP domains. Assists women and their families in establishing goals and outcomes. Provides education, support and community referral resources to assist in attaining targeted goals, including smoking cessation. Plans home visits in accordance with client goals and NFP outcomes. Evaluates client progress toward stated goals and NFP outcomes. Communicates with other health professionals and referral sources to ensure receipt of services and individualized care. Understands, supports, and coaches others in the NFP vision, mission and model. Maintains clear, effective, open, honest communication with both internal and external customers and co-workers. Maintains confidentiality of client information. Schedules joint home visits with NFP Nurse Supervisor every four months. Participates in weekly supervisory sessions utilizing reflective practice to review client progress with the supervisor.

 

Documents case management activities and maintains electronic client health records. Completes Encounters and EARS daily to document direct services, risk factors, referrals and other interventions. Documents within 48-72 hours of service.  Completes required NFP forms and records data for input into Nurse-Family Partnership database. Meets work standards and programmatic goals and objectives. Maintains standard-applicable regulations for personnel, medical records management and programmatic/funder requirements. Reviews and responds to email daily.
