Under the direction of the Clinical Pharmacy Manager, the Clinical Pharmacist is responsible for providing comprehensive medication therapy management services, with a primary focus on chronic disease state management. Working collaboratively with providers and care teams, the Clinical Pharmacist optimizes medication use, monitors clinical outcomes, orders and evaluates medication-related laboratory tests, identifies and resolves medication-related problems, and empowers patients through education and self-management support.
Duties and Responsibilities:
Chronic Disease Management Services (Primary responsibility)
Conduct comprehensive chart reviews utilizing OCHIN Epic and pharmacy systems to assess medical history, diagnoses, laboratory results, medication regimens, allergies, and other relevant clinical information.
Evaluate, initiate, modify, and optimize medication therapy in accordance with established protocols and collaborative practice agreements, including monitoring patient outcomes, medication adherence, adverse effects, drug interactions, disease-state control, and appropriate laboratory parameters
Provide patient-centered education and counseling to support medication adherence, self-management, and healthy lifestyle modifications.
Specialty Pharmacy Services
Support the Specialty Pharmacy Program by coordinating specialty medication management from referral through ongoing follow-up, including patient onboarding, education, care planning, and reassessments.
Monitor medication adherence, therapeutic response, adverse effects, and medication-related problems, and collaborate with providers to optimize specialty medication therapy.
Maintain accurate documentation and ensure compliance with accreditation, manufacturer, payer, and organizational requirements
Medication Refill Services
Review, evaluate, and process (Approve, modify, or deny) medication refill requests in accordance with SCC refill protocols with timely processing and documentation.
Identify medication-related concerns and communicate recommendations to providers.
Assist with prescription clarification, electronic prescribing issues, and prior authorizations.
Additional Clinical & Operational Responsibilities
Participate in quality assurance, patient safety, and workflow improvement initiatives.
Troubleshoot operational and clinical issues and escalate to the Clinical Pharmacy Manager as appropriate.
Maintain current clinical knowledge through continuing education and professional development activities.
Position Requirements:
A Doctor of Pharmacy (Pharm.D.) degree required
PGYl(Community or Ambulatory) residency training or equivalent clinical experience (One to two years prior experience in a similar position providing Medication Therapy Management in a community clinic setting, particularly working with patients with Diabetes, hypertension, and hyperlipidemia (Required)
California Advanced Practice Pharmacist License, and or board certification (Highly Preferred)
Fluency in written and spoken English , bilingual highly preferred.
Current BLS/CPR certification is required
Experience using an electronic health record system, preferably OCHIN Epic (preferred)
Demonstrated ability to work as a member of a multi-disciplinary team of staff and volunteers and to be able to work with professionals with different management styles
Experience managing chronic disease states such as diabetes, hypertension, hyperlipidemia, asthma/COPD, anticoagulation, or obesity-related medication therapy
Ability to interpret clinical data, laboratory results, and medication histories to support evidence-based medication therapy decisions
Strong patient counseling, motivational interviewing, and health education skills
