Description
The BMSC Adult Community Clinical Services (ACCS) Program is seeking a dynamic candidate who will serve as a Licensed Practical Nurse on one our integrated teams. The LPN II will provide supports & services to adults with major mental illness in Department of Mental Health ACCS Group Living Environment residential homes as well as in the community. The LPN II will provide face-to-face community-based services to assigned persons in their homes, homeless shelters, in hospital settings or any other setting in which the person served resides or currently requires intervention (including in one of the BMSC ACCS 3 24/7 group residential environments). The LPN II will provide behavioral and physical health monitoring and support as well as medication administration and education as needed. The LPN II will be responsible for completing Physical Health Assessments, participating in the development of treatment plans, and providing trauma-informed, person-centered community-based interventions that target skill development in the areas of Health & Wellness. The LPN II will conduct routine assessments and provides medical care as needed, while also ensuring that appropriate medical services are accessed by collaborating with care coordination entities.
Please Note: This position is 40 hours per week, Monday to Friday from 8:30 am to 5:00 pm. The scheduled days off will be on Sunday and Saturday.
Employees in this position will be required to perform mandatory overtime.
Duties and Responsibilities (these duties and responsibilities are not all inclusive):
- Serves on an ACCS Integrated Team by attending regularly scheduling meetings, by coming prepared to present psychosocial history, mental status, diagnosis, treatment provided and the treatment planned for assigned cases and by participating in discussion of other cases so as to ensure that persons served receive optimal care.
- Assists in the development, understanding and implementation of treatment plans of those that are on assigned case load by involving person served and significant others as appropriate.
- Assess on an ongoing basis the needs of assigned persons served by providing supportive counseling, by reviewing records, by making home visits and by maintaining regular contact with persons served and families in order to ensure that appropriate treatment recommendations are made.
- Functions as an advocate and liaison for persons served in interactions with the persons served’s primary and other health care providers, conferring as necessary regarding medical and medication issues and adjustments.
- Provide rehabilitative interventions to assist persons served in building and using skills and accessing resources to achieve treatment goals, manage symptoms and live productive and meaningful lives. These interventions take into account SAMHSA’s 8 Dimensions of Wellness.
- Read and communicate all pertinent information and attending daily rounds, staff meetings and informing supervisor or designee of any critical information.
- Transport or arrange transportation for persons served that need that level of service.
- Assess persons served mental status and assist them with utilizing coping strategies, natural supports and safety planning when necessary.
- Involves all assigned persons served and significant others, when appropriate, in the process of treatment planning and ensure that plans are agreed upon by obtaining persons served or LAR signature.
- Maintain person served records in a timely manner by documenting in EMR and paper record and completing all other related forms to ensure compliance with DMH regulations and Rehab Option requirements.
- Provides primary nursing care by assessing health status, recording related health data, evaluating outcomes and adjusting care in conjunction with other service providers.
- Transcribes and ensures implementation of physician’s orders.<
