Medication History Pharmacy Technician

Rivers Health is seeking a fulltime Medication History Pharmacy Technician, to perform the essential functions below. 

 

The Medication History Pharmacy Technician is responsible for obtaining accurate home medication lists from patients entering the hospital and conveying that information to the appropriate provider of care. When necessary, under the supervision of a registered pharmacist this position will also assist pharmacists in preparing and distributing medications. Pharmacy technicians work only under the supervision of a registered pharmacist.  They do not perform duties that can legally be performed only by a registered pharmacist.

ESSENTIAL JOB FUNCTIONS: 

This job description outlines the essential functions of a Medication History Technician, emphasizing the role’s critical involvement in gathering and verifying patient medication histories. 

1. Medication History Collection 

• Gather accurate home medication lists from patients upon hospital admission. 

• Communicate collected data to appropriate healthcare providers. 

1. Work Environment 

• Operate in multiple hospital areas, including ER, Same-Day Surgery, Preadmission Testing, and Inpatient Units. 

1. Verification & Reconciliation 

• Use standardized interviews, phone calls, pharmacy records, and state databases to confirm medication histories. 

• Update electronic records and reconcile medication lists with provider orders. 

1. Pharmacist Collaboration 

• Discuss high-alert medications with the pharmacist as needed. 

• Notify the pharmacist immediately if discrepancies arise after provider medication reconciliation. 

1. Quality & Documentation 

• Conduct quality monitoring and data collection. 

• Ensure proper documentation of medication histories and reconciliation activities.

QUALIFICATIONS: 

The following qualifications, or equivalents, are the minimum requirements necessary to perform the essential functions of this position. 

  

EDUCATION AND FORMAL TRAINING: 

• High school diploma or GED
• Certified Pharmacy Technician (CPhT) required
• ASHP Medication History-Taking Certificate or the Pharmacy Technicians  Certification Board’s (PTCB) Medication History Certificate, within the first on
