Responsibilities & Requirements
JOB SUMMARY
The Medication Management Coordinator position is responsible for supporting all patients to ensure the prescribed drug therapy and pharmacy care plan are followed as prescribed and also meet the medical, social and personal needs of the patient. The goal is to maximize the overall effectiveness of the medication and improve patient outcomes.
KEY JOB RESPONSIBILITIES:
- Work with patients to help overcome financial barriers that prevent medication access
- Insure prescribed medications are obtained by patient
- Assist Ambulatory Care Pharmacist in maintaining an accurate medication regimen.
- Routinely call patients to address drug related questions and identify access and adherence barriers.
- Assist Ambulatory Pharmacy as needed.
REQUIREMENTS:
Education:
- High School diploma or equivalent
Skills and Experience:
- Must have understanding of retail pharmacy process in obtaining medications
- Must have understanding on pharmaceutical manufacturers medication assistance programs
- Must be competent and proficient to use computers and applications, understand and utilize the electronic medical system, and related equipment within the pharmacy.
- Must be able to prioritize work to assure that most emergent situations are handled first,
- Must be able to collaborate and communicate professionally with the staff, patient family members, patients, physicians, outpatient pharmacies, and other healthcare personnel both effectively and efficiently daily.
- Must possess strong oral and written communication and interpersonal skills.
- Must uphold and complies with Corporate HIPAA regulations, all Institutional and KHN policies and procedures, and Pharmacy Departmental policies and procedures. Must complete all mandatory education and competency required annually. Must comply with KH Standards of Behavior.
Licensure, Certifications, Registrations:
- State of Ohio Pharmacy Technician Certification
- Pharmacy Technician Certificate required or willing to get Pharmacy Technician Certificate within two years of employment.

There may be periods of meeting deadlines with severe time constraints, often with multiple demands from several people.
