Position Summary

• Responsible for assisting the Director of Community Health in the Community Benefits in coordinating needs assessment to develop and implementing regionalized community benefit programs while advocating for the optimal health and well-being of the communities we serve.

Principal Accountabilities

Education

• Bachelor’s degree in Public Health, Health Education, Social Services, or related field.

Licenses and Certification

• Certified Health Education Specialist (CHES)

Required Experience and Skills

• Technical Experience:  2 years in coordinating public health benefits required.
• Knowledge of Healthy Southern Illinois Delta Network Resources.
• Experience with coalition building and engagement.
• Coordination of community events and screenings
• Knowledge of Community Health Needs Assessment (CHNA) process.
• Managing receipt of and monitoring mini-grant programs for community-based organizations.
• Stages of Change Theory in program development and delivery.
• Ability to present on a variety of health topics in collaboration with community- based organizations.

Physical Activities

• Intermittent hand manipulation required
• Intermittent lifting and carrying of 20 pounds

Role Specific Responsibilities

• Assists in the development and implementation of the community health needs assessment.
• Plans and implements effective health promotion and education programs.
• Coordinates and communicates with local and state programs.
• Assists the Director of Community Health with managing receipt of and monitoring mini-grant programs for community based organizations
• Performs public health duties through various community resources.
• Coordinates community events and screenings.
• Develops promotional materials.
• Manages and develops grants as applicable.
• Utilizes Lean to optimize initiatives.
• Perform various speaking engagements and presentations to represent the organization in a positive light and to improve health and community awareness.
• Utilize Stages of Change theory in program development and delivery.
• Advocate within the community for policy, system and environmental changes to increase opportunities for optimal health.
• Promotes and advocates for various programs such as the Medical Legal Partnership of Southern Illinois. 
• Build relationships within the community to encourage collaboration.
• Develop and maintain healthy community coalitions and action teams which support health programming.
• Knowledgeable about the Healthy Southern Illinois Delta Network resources.
• Must demonstrate ability to collaborate with multiple community partners/organizations.
• Serves as the SIH Program Coordinator for the Stanford Diabetes/Chronic Disease Self-Management program throughout southern Illinois.
• Directs or facilitates the implementation of evidenced based efforts to achieve measurable outcomes in improving community health.
• Demonstrates leadership, the ability to work independently, and carry our strategic and operation plans.

Compensation (Commensurate with experience):

$52,187.20 - $80,891.20
