The Home Care Rehabilitation Services Team Leader is responsible for overseeing appropriate practice and compliance by rehabilitation therapists (PT, OT, SLP). They will be a resource person by providing subject matter expertise and quality practices for rehabilitation home care services and provide education and training, as applicable. This position also assists with staffing and scheduling. When needed, this person will provide direct patient care and follow the guidelines as indicated in the job description of their applicable job title. The Team Leader provides feedback to leadership related to team member and departmental performance.
RESPONSIBILITIES (ESSENTIAL FUNCTIONS):
- Oversees daily operations and coordinates staff scheduling and delivery of patient care.
- Provides direct patient care in accordance with program expectations.
- Provides input related to productivity, service delivery, cost containment, and operational procedures.
- Prioritizes tasks and goals, maintains quality, and completes their work within the expected, appropriate time frames.
- Plans and conducts in-services and training programs on therapy and related topics as needed for educational, medical, and other professional groups as applicable.
- Adheres to OK State Medical Board Licensure and Supervision (OSMLBS) general and direct supervision requirements and provides consultation as needed.
- Performs as a clinical educator and completes competency checklist as established per departmental standards.
- Mentors and supports clinicians by leading, engaging, and motivating them to provide exceptional care while constantly improving processes outcome and patient satisfactions.
- Proactively identifies opportunities to improve department processes/issues, consider various alternatives, and recommends the appropriate course of action.
- Assists in the collection and analysis of data concerning inter/intradepartmental quality improvement processes/issues within the designated time frame.
- Reduces and controls environmental hazards and risks by actively participating in identification of potential hazards and recommending effective changes to assist in avoiding future dangers.
- Maintains conditions conducive to optimal safety and well-being of all patients and team members by demonstrating critical thinking skills to assure positive outcomes.
- Demonstrates understanding of the obligation to preserve the confidentiality of patient and team member health and financial information.
- Demonstrates essential aspects of teamwork by approaching relationships in a cooperative manner to reduce conflicts and minimize problems.
- Maintains safe and clean working environment by complying with procedures, rules, and regulations, and adhering to infection-control policies and protocols
- Demonstrates leadership skills by actively participating in task force and committee roles as assigned.
- Regular attendance and punctuality for scheduled shifts.
- Appropriately adapts assessment, treatment, and/or care methods to accommodate the physical, cultural, age-specific, and other developmental needs of each person served.
- Maintains professional and technical knowledge through continuing education opportunities including internal and external educational offerings.
- Utilization of assistive devices for lifting is mandatory.
- Must adhere to, and promote, safety protocols at all times.
- Per DRH policy, all required conditions of employment must be met and maintained including required vaccinations.
