About the role
Primary Accountability
The Licensed Marriage and Family Therapist (LMFT) is responsible for providing primary care medical services in a Patient-Centered Health Home context, focusing on access to care.
Description of Primary Responsibilities:
- Responsible for directly providing behavioral health services and meeting all established productivity expectations.
- Conduct comprehensive clinical assessments to identify patient needs and develop individualized treatment plans.
- Provide individual, couple, and family therapy sessions utilizing evidence-based therapeutic modalities.
- Facilitate open communication and conflict resolution within families and couples.
- Expected to document visits accurately utilizing appropriate templates and record relevant data to attain a diagnosis.
- Accountable for attaining budgeted visits.
- Accountable for meeting clinical care measures.
- Accountable for locking records within a72-hour window.
- Accountable for providing excellent customer service and care.
- Consistently behaves courteously when interacting with patients/ family members, support staff, and Network staff.
- Provider remains aware of wait time during the daily workflow.
- Responsible for providing the patient with educational materials and resources appropriate to the patient’s health literacy needs.
- Accountable for patient status as measured by relevant clinical quality measures.
- Promotes and participates in daily huddles.
- As an ancillary health team member, participate in managing the care of patients/ families through individual visits and group classes.
- Responsible for setting and monitoring self- management plans.
- Responsible for assisting in the assurance of clinical procedures and the maintenance of up-to-date clinical protocols.
- Completes peer review responsibilities in a timely manner.
- Assists the patient care team with performance improvement efforts, staff education, and FHCN promotion.
- Responsible for assisting the patient care team with performance evaluations and quality improvement.
- Where relevant, participate in supervising pre-licensed behavioral health staff.
- Participates in Network Health Fairs & other community events.
- Participates in site & department meetings in a meaningful way.
- Meets Network and credentialing expectations for licensure and employment.
- Responsible for adhering to the Attendance and Absenteeism Policy, recognizing that regular attendance is considered an essential function of all FHCN positions. Absenteeism is not being at work or failing to attend a paid workshop, training, or event unless the absence is protected by law.
- Ability to present to and work at any FHCN location, both at the beginning of a shift or during a shift, based on business need.
- Performs other duties as assigned.
Description of Primary Attributes
Professional & Technical Knowledge:
Effective June 1, 2026, all individuals hired into the role must:
- Possesses specific advanced knowledge and skills, including written and verbal communication skills, computational, computer, and technical skills, and mathematical knowledge, frequently acquired through one of the following:
- Completion of a Master’s degree program with a recognized major with a minimum cumulative GPA of 2.5; or
- Successful attainment and maintenance of a valid, active professional license required for the role, issued by the appropriate governing or regulatory body.
- Performance of the duties and responsibilities of the job requires the equivalent of extensive formal training in behavioral health, including an understanding of the application of the theory and practices of the profession to the operations of the organization, usually in the form of a major in Social Work, Psychology, or Marriage and Family Therapy as part of a Master’s degree.
Technical Skills:
- Ability to create highly complex documents in Microsoft Word, including linking multiple files and embedding objects linked to other documents.
- Ability to use advanced functions of Microsoft Excel, such as creating and managing databases, including creating standardized reports or linking multiple worksheets and workbooks.
- Ability to develop sophisticated presentations in Microsoft PowerPoint, including embedded objects, transitions, and other elements.
Licenses & Certifications: Licensed as a Licensed Marriage and Family Therapist by the state of California.
