Pay Rate: $38 – $42 per hour (DOE)
Schedule: Full-Time or Part-Time

Willow Creek Healthcare Center is seeking a skilled and compassionate Physical Therapist Assistant (PTA) to join our rehabilitation team. We are a skilled nursing facility committed to providing excellent patient-centered care and fostering a supportive work environment for our staff.

Key Responsibilities:

• Deliver physical therapy treatments as directed by the supervising Physical Therapist

• Accurately record daily treatment notes and weekly progress reports per PT Board standards

• Assist in department upkeep and equipment maintenance

• Participate in care planning and interdisciplinary rehab conferences

• Collaborate with nursing staff to train Restorative Aides

• Communicate effectively with healthcare team members regarding patient care plans and progress

• Conduct in-service training for facility staff as needed

• Educate patients, families, and caregivers in preparation for discharge

• Ensure documentation meets all facility and regulatory requirements

Qualifications:

• Licensed as a Physical Therapist Assistant in the state of California

• Experience with Casamba and PointClickCare preferred

• Strong written and verbal communication skills

• Knowledge of therapy best practices and documentation standards

• Must maintain continuing education per state board guidelines

• Ability to lift/push/pull up to 100 lbs occasionally and meet physical demands of the role

• Ability to relate positively and professionally with patients, families, and team members
