About the role
<p>1. Provides patient care within a designated shift, based on the nursing<br/>process and within role/scope of practice</p>
<p>2. Provides direct patient care, as outlined in the patients’ plan of care and/or as directed by the Registered Nurse, to promote comfort and/or return to function health for patient.</p>
<p>3. Provides care based on physical, psycho/social, educational, safety and related criteria, appropriate to the ages of the patients served.</p>
<p>4. Obtains information to contribute to an accurate and current patient assessment, patient care plan and discharge plan.</p>
<p>5. Includes the patient and caregivers in this process to obtain accurate information /history and contributes to the RN’s assessment.</p>
<ol>
<li>Delivers compassionate care to patients and supports members of the patient’s family while providing patient care in an organized manner, utilizing knowledge of nursing process-assessments, planning, intervention, and evaluations.</li>
</ol>
<p>7. Communicates effectively and professional with all patients, guests, family members and co-workers to ensure a quality patient experience.</p>
<p>8. Adapts to rapid change in appositive manner throughout the shift as the needs of the department change.</p>
<p>9. Communicates effectively with physicians and team members about changes in patient’s clinical conditions, including results of diagnostic studies and symptomology.</p>
<p>10. Ability to respond quickly and accurately to changes in condition or response to treatment.</p>
<ol>
<li>Performs designated therapeutic procedures to assigned patients as<br/>needed.</li>
</ol>
<p>12. Accurately observes, records and reports to the Registered Nurse, all<br/>patient concerns, symptoms, reactions and progress in a timely manner.</p>
<ol>
<li>Contributes to the nursing assessment by collecting, reporting and<br/>recording objective and subjective data in an accurate and timely manner.</li>
</ol>
<ol>
<li> Documents nursing interventions and responses to care, communicating<br/>to appropriate members of the health care team</li>
</ol>
<ol>
<li>Provides individualized health education to the patient and caregivers to specifically meet the patient’s needs related to the current diagnosis.</li>
</ol>
<p>16. Assists with admitting, discharging and transferring of patients.</p>
<p>17. Identifies and addresses psychosocial needs of patients and family; communicates with Social Services/Discharge Planner regarding both in-hospital and post discharge needs.</p>
<p>18. Problem Recognition/Management: In response to data indicating potential/actual risk to individual's health, initiates action to correct, reduce or prevent the risk in collaboration with the registered nurse gives a rationale for actions or decisions based on, successful experience, skills and knowledge or standards of practice/protocols. Contributes to the development of a plan of care for assigned patients in collaboration with the registered nurse</p>
<h1>19. Collaborates with interdisciplinary team members to identify actions to improve patient care.</h1>
<h1>20. Seeks supervision, consultation, assistance when unable to perform effectively or safely independently.</h1>
<p>21. Initiates and/or maintains interventions to assure continuity of safe/effective care of patients including medication management and patient instruction.</p>
<h1>22. Uses interpersonal-communication strategies with individuals that are intended to achieve desirable/acceptable outcomes and a perception of satisfaction by those involved.</h1>
<h1>23. Uses internal and/or external resources to resolve and/or prevent problems that cannot be managed independently.</h1>
