The Care at Home Registered Nurse remotely monitors and navigates patients and families across the continuum of the Medically Home phases of care. The Care at Home Registered Nurse performs assessments, monitoring, planning and interventions for acute care patient populations in the home while using in-home technology and protocols in collaboration with other members of the health care team.
The ideal caregiver is someone who:
• Is able to work onsite in Vero Beach, FL.
• Has three years of RN hospital experience strongly preferred.
• Has strong verbal and visual assessment abilities as this position will be providing virtual care to Medical/Surgical level patients.
• Can critically think and problem solve for patients’ best interests.
Schedule Requirements:
Night Shift, 7pm - 7am
Full Time: Three 12 Hour Shifts Per Week
Rotating Weekends and Holidays
A caregiver who excels in this role will:
Provide direct nursing care.
Implement, monitor, evaluate, update and revise patient care plans.
Monitor, record, document and communicate patients’ conditions.
Administer prescribed medications and treatments.
Note and carry out physician and nursing orders.
Assess and coordinate patients’ discharge needs.
Educate patients and their families.
Minimum qualifications for the ideal future caregiver include:
Graduate from an accredited school of professional nursing.
Current state licensure as a Registered Nurse (RN).
Basic Life Support (BLS) certification through the American Heart Association (AHA).
One-year of current acute care RN experience. Will consider licensed military RN experience.
Preferred qualifications for the ideal future caregiver include:
Bachelor of science in nursing (BSN).
Three years of inpatient RN experience.
Experience working in a virtual environment.
