Registered Nurse, OB/Perinatal, Rainier Beach (.6 FTE)

Purpose

The Perinatal Program RN works in partnership and joint accountability with other staff members to achieve Neighborcare’s Mission, Guiding Principles and goals. The primary focus of the RN is providing appropriate care for the antepartum/postpartum OB population in a multi-cultural setting. The RN provides direct patient care, follow up, coordination of high risk patients, and complex case management. The Perinatal Program RN facilitates coordinated care and collaboration with all care providers including Maternity Support Services, OB Care teams, and specifically serves as the primary point of care continuity for the Residency Teams. While RNs will be assigned to a specific clinic site, they will also work in partnership with each other to meet the needs of patients throughout Neighborcare. The ANA Standards of Clinical Nursing Practice and the AAACN Ambulatory Care Nursing Administration and Practice Standards provide a basis for the practice of the registered nurse in the ambulatory setting. The Perinatal Program RN follows the nursing process to assess needs, plan care, implement the plan, and evaluate the patient's responses and progress towards goals. In this position you will: 

Health Promotion & Case Management (40%)

• Develop a proactive model of care to assist providers in anticipating milestones and utilize practice guidelines to ensure appropriate follow-up
• Utilize the electronic health record to communicate with members of the care teams and implement care plans in person or by telephone according to Neighborcare Health Tasking Guidelines
• Communicate and collaborate with community partners including Public Health, UW, cultural organizations and others serving and supporting patients
• Orient and support teaching of rotating residents to Neighborcare Health and the patient population
• Facilitate care coordination between outside care providers and internal clinic team to insure care providers are aware of the patient’s current condition, needs and medical management priorities (through EHR or case conferences)
• Develop partnerships with patients and families and facilitate continuity of care between patients and OB care providers (residents) for positive outcomes in health status
• Identify patient and family goals for improving health status
• Assess patient and family’s understanding of health and readiness to change
• Provide patient and family with education about development and disease processes and offer tools and skills needed to achieve self-management goals
• Coordinate care according to EHR (Electronic Health Records) OB templates
• Monitor, track, follow-up with patient and family as needed
• Follow Neighborcare Health protocols and utilize resources of best clinical practices to advise patients and families
• Coordinate site-based Centering Pregnancy programs. We currently offer Centering in English, Spanish and Somali. This role may include being directly involved in the facilitation of the group care, monitoring and reporting of outcomes to the Centering Healthcare Institute, ongoing quality improvement and engagement with CHI Consultants, overseeing scheduling and organization, purchasing of supplies, outreach and further development of the program, grant acquisition/management and facilitating the Steering Committee and regular meetings
• Partner to ensure smooth process for intake appointments and counseling on care models

Patient and Family Education (30%)

• Educate patients and families in the use of medications
• Educate patients and families regarding abnormal laboratory results requiring education and follow-up
• Provide patients and families information to assist with self-care
• Promote wellness and the importance of preventive care
• Provide motivational interviewing and harm reduction counseling
• Provide gestational diabetes teaching including use of glucometer, monitoring blood sugars, warning signs, etc.
• Perform health screenings and assessments (mental health, DV, etc) as indicated
• Assist patients with the development of birth plans
• Refer clients to outside resources (L&D classes, vision resources, home visit nurse, etc)
