The Center is a 501(c)(3) nonprofit community organization that assists people without regard for religious affiliation or lack thereof through comprehensive case management and integrated social service programs. The Center enjoys broad-based community support as well as a sponsored relationship with its founders, the Sisters of St. Joseph of Carondelet. St. Joseph Center serves approximately 6,000 individuals annually.

Multifaceted intervention, prevention, and education services are carried out at four sites on the Westside of Los Angeles. St. Joseph Center’s integrated programs provide clients with concentrated and coordinated access to services according to the nature of their needs.

Job Summary
In partnership with the Los Angeles County Department of Mental Health through the Behavioral Health Bridge Housing (BHBH) Program, St. Joseph Center will provide interim housing opportunities designed to meet the immediate needs of individuals experiencing homelessness who are living with behavioral health conditions and/or substance use disorders. The BHBH Program offers a safe, private, and secure environment where participants are empowered to stabilize, build self-sufficiency, and transition from homelessness to permanent housing.
Under the supervision of the Program Manager, the Case Manager II is responsible for providing direct client support and performing the duties outlined below.
ESSENTIAL DUTIES & RESPONSIBILITIES
Client engagement & Assessment
- Conduct thorough intake interviews with new clients to clearly communicate program guidelines, policies and procedures, ensuring clients fully understand their rights, responsibilities, and the range of supportive services available to them.
- Build trust and rapport with clients while maintaining professional boundaries.
- Utilize trauma informed, harm reduction, and housing first methodologies when working with clients.
- Ensure the safety and well-being of all residents by implementing and monitoring House Rules and regulations.
CASE MANAGEMENT & FOLLOW UP
- Develop individualized service plans focused on housing stability, behavioral health, substance use support, employment, and life skills.
- Meet regularly with clients to monitor progress, revise goals, and offer support.
- Assist with life skills development, including budgeting, time management, and conflict resolution.
- Develop and implement an individualized housing and services plan that supports housing goals, increases income and mainstream benefits, supports employment opportunities, and addresses mental health, physical health and substance use needs, relapse prevention, and any other risk factors that could impede accessing and/or maintaining permanent housing.
- Provide ongoing case management services that will ensure successful permanent housing, decrease social isolation and reduce relapse risks, link to any needed services such as medical, dental, mental health, substance use, life skills training, money management, meaningful community activities, volunteer services, job placement, etc. in tandem with on-site team.
- Obtain and retain all required documentation (e.g., ID, income verification) as required by the program.
- Provide crisis intervention and de-escalation when necessary.
COLLABORATION & TEAMWORK
- Participate in case conferences, staff meetings, and multidisciplinary team discussions.
- Work closely with resident monitors, housing navigators, mental health professionals, and outreach workers to ensure coordinated care.
- Report concerns regarding client safety, mental health, or habitability to appropriate personnel.
- May help train and mentor new case managers.
DOCUMENTATION & COMPLIANCE
- Maintain accurate and timely documentation in client records and electronic databases (HMIS); all case notes entered in GIRP format in HMIS within 24 hours.
- Complete service plans, progress notes, incident reports, and outcome tracking as required.
- Ensure compliance with funding or program requirements, including privacy and confidentiality standards (e.g., HIPAA).
