Job Overview
The Nursing Director of AltaMed Plus position is responsible for oversight of care coordination and care management services to AltaMed senior patients, interdisciplinary team meetings, back office management, and others. The Nursing Director supports the program which was developed for our geriatric members requiring a more personalized experience in their care management. The Director supports other organizational focused efforts around senior care and serves as the triad partner to the Associate CMO/VP of Senior and Complex Care and Program Director of Senior and Complex Care, to provide leadership oversight over a transformative health care delivery model with personalized care, and emphasis on chronic condition management; to help seniors and those facing complex illness achieve their health goals. In addition, the Nursing Director works collaboratively with key organizational partners to deploy standard operational process, performance measures, priorities, and policies to achieve quality health outcomes. Lastly, The AltaMed Plus Nursing Director serves as an integral member of the care team, assesses patients for social determinants of health (SDOH), risk of adverse health outcomes, inappropriate utilization, and monitors the impact of care management interventions.
Minimum Requirements:
1. Bachelor’s degree in Nursing required.
2. Master’s degree in Nursing preferred.
3. Registered Nurse with a current California State Board of Nursing License.
4. Minimum of 7 years of Registered Nursing experience required. Ambulatory Care, Acute Care, or Case Management experience preferred
5. Minimum of 2 years of supervisory experience required.
6. Bilingual English/Spanish preferred.
7. BLS certification. ACLS certification preferred.
8. Training and certification in Hospice and Palliative Care and/or Geriatric Medicine highly valued

Salary Range
