Compassionate In-Home Private Duty LPN - Full-Time, Day Shifts

 

Location: Powell, TN (Patient's Home)

Schedule: Monday to Friday from 9:00 AM to 5:00 PM

 

Job Description:

Join our team at Adoration Health as a Licensed Practical Nurse (LPN) for a fulfilling full-time private duty nursing role in the Powell, TN area. This position involves providing specialized care, including g-tube and incontinence care, to a patient in their home environment.

Key Responsibilities:

• Deliver compassionate, personalized care to the patient.
• Manage g-tube care as required.
• Provide incontinence care and assist with daily living activities.
• Administer medications and treatments as prescribed.
• Monitor patient’s health and update medical records.
• Collaborate with the healthcare team to provide comprehensive care.

Requirements:

• Current LPN license
• Experience with g-tube and incontinence care is preferred
• Strong communication and interpersonal skills
• Ability to work independently and manage time effectively

Responsibilities

• Perform selected nursing procedures within the scope of LPN practice
• Provide patient and family teaching according to plan outlined by the RN
• Monitor patient's vital signs and make specific measurements and observations
• Monitor and record intake and output. Report significant changes immediately
• Observe, assess and record patient's physical and mental condition and reports any changes
• Administer medications as instructed and as ordered by the physician and instructed by the RN and document the patient's file accordingly
• Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned

Qualifications

• Licensed Practical Nurse / LPN license in good standing
• Previous private duty, home health, hospice, hospital or skilled nursing experience preferred
• Active CPR certification
• Familiarity with electronic health records
• Strong problem solving and critical thinking skills
• Reliable transportation, valid driver's license and current automobile insurance
