Sentara is looking for a full time Clinical Pharmacist to join the Home Infusion team in Charlottesville.

Shift: Mon - Fri (8am - 5pm) w/ on call rotation

The Home Infusion - Clinical Pharmacist works in a high volume infusion pharmacy within the homecare setting.

The Home Infusion Clinical Pharmacist:

  • Assures the appropriateness of all physician medication orders.
  • Manages patient drug therapy in accordance with the medical staff approved clinical practice standards.
  • Maintains current knowledge of drug use through continuing education.
  • Serves as the primary resource for drug information within home infusion pharmacy.
  • Serves as a resource for community health education.
  • Precepts pharmacy students as assigned.

Qualifications:

  • Pharmacist License with VA Board of Pharmacy required
  • Bachelor's Level Degree OR PharmD from an accredited school of pharmacy required
  • IV Room and Hospital experience preferred

Job Summary

The Home Infusion Clinical Pharmacist works in a high volume infusion pharmacy within the homecare setting. The clinical pharmacist assures the appropriateness of all physician medication orders. Registered Pharmacist License in the Commonwealth of Virginia or eligible for license required.
