Under the Division Director, the Floating Case Planner will assist case planners within the Division with day-to-day social services tasks. They will be responsible for supporting constructive services to children in foster care that improve their functioning. Connect parents to services that address family challenges and assist them with reuniting with their child. Their assignment can include filling in temporarily for a vacant case planner position to supporting their colleagues with family visit coverage, school visits, safety contact, etc.
Primary Responsibilities and Essential Functions of Positions:
- Creates, in partnership with child, biological family and foster family, service plans which address safety, behavioral, educational, psychological and permanency needs.
- Monitors safety, well-being and progress against other critical outcomes by facilitating regular casework contacts and by documenting the same in progress notes and other case documentation.
- Schedules and supervises mandated visits between child and natural family; conducts ongoing required interviews in person with child, natural family and foster family; independently makes all related field visits, to homes, schools, courts, prisons, hospitals and other sites as case may require.
- Attends weekly, at minimum, supervision with the supervisor to present and discuss case situations and provide update on status of current work; carries out supervisor directions regarding planning and service follow up.
- Attends all Family Court hearings on assigned cases; provides testimony regarding case status and Agency position; follows up on court directives
- Participates actively in Family Team Conferences and other meetings to advance permanency for children and families.
- Works closely with Integrated Services (Medical, Mental Health and Case Management) to ensure that the medical and mental health needs of children and the biological family, are being monitored appropriately, necessary medical training is being received and provided to foster parents and birth parents, services are being provided according to treatment needs and external collaborations are occurring as needed in order to ensure safety, permanency and wellbeing
- Utilizes individual and unit data collected by the city, state, federal and agency data and accountability departments, in order to measure/monitor performance and ensure that all outcomes are being met
- Serve as an alternate support coverage by facilitating the Family Team Conference (FTC).
Sign-on bonus of $2,000
Qualifications
- Bachelor's degree required.
- Knowledge of Child Welfare System is preferred.
- Excellent written, oral, organizational, and interpersonal communication skills, and be a collaborative team player
- Proficiency in Microsoft Office, including Outlook and Excel.
- Computer proficient, knowledge of Connections.
- Ability to use public transportation to travel throughout New York City or Agency vehicle when available.
