Job Summary
Responsible for the implementation and support of Structural Heart programs as assigned with a multidisciplinary team of physicians and clinical staff. Coordinates all program functions and aspects of patient care, from promotion and development of a referral base to the clinical assessment of patients, planning of Structural Heart and associated procedures, and formulation of long-term care plans.Essential Functions

Promotes direction of the Structural Heart program as established by the Strategic Leadership team of Heart & Vascular Institute. Establishes a referral base by building partnerships with internal and external physicians, customers, agencies and vendors.
Manages the multidisciplinary plan of care for Structural Heart patients to optimize the patients' clinical and functional status. Ensures continuity of care by coordinating all admissions, managing patient flow, monitoring outcomes, and communicating with the Structural Heart team.
Serves as Structural Heart subject matter expert and community liaison. Respond to program inquiries and referrals, build awareness, and provide excellent customer service.
Provides clinical consultation and education to patients, families, staff, and other disciplines that have contact or care responsibilities for the Structural Heart patient population.
Evaluates patients to confirm eligibility appropriate for identified Structural Heart procedure. Accesses internal/external medical records, analyze clinical data, document risk factors, arrange diagnostic testing, and compile information for physician review.
Plans long-term follow-up care of Structural Heart patients and compile treatment options. Provides patients with educational reinforcement and feedback on their progress. Provides clinical care to include office visits, admission/discharge orders and planning.
Works in partnership with Structural Heart physician leaders. Facilitate effective communication between disciplines and employ an integrated approach to patient care.
Maintains the program-associated registries. Collects and submits required patient data. Maintain documentation to meet regulatory standards and ensure compliance with applicable policies and guidelines.
Conducts quality assurance / improvement assessments as needed. Assists with development of standard protocols and procedures for the Structural Heart program to optimize outcomes.
Completes other duties including assistance with research registries, clinical trials and regulatory tasks.
Performs other duties as assigned.
Supervisory/Management Responsibilities
This is a non-management job that will report to a supervisor, manager, director or executive.
Minimum Requirements
Education - Associate degree in Nursing. Bachelor's degree in Nursing preferred.
Experience - No experience required. Experience in multidisciplinary clinical care atmosphere preferred Supervisory/Management experience - preferred
