Compensation Range:

$76,398.00 - $99,320.00

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Responsibilities:

Provides individualized assistance, support and comprehensive coordination of care for patients and their families/caregivers, to eliminate barriers to timely care, facilitate flow through system, facilitate interactions with care team, increase patient and provider satisfaction, maintain point of contact with providers and serve as first point of contact for patients and families.

Schedule FT position available Responsibilities The Nurse Navigator will play a crucial role in guiding patients through the entire continuum of care related to joint replacements. The Navigator is a key member of the multidisciplinary team helping to remove barriers for patients and provide resources for pre and post-surgical care. This role ensures defined standards at every episode of care for screening, assessment, planning, patient education, managing and communicating to assure the progression of safe and efficient care throughout the continuum in a collaborative approach. They are responsible for the data collection on defined metrics and will take initiative to identify gaps in the continuity of care for patients undergoing joint replacement surgery. Nurse navigators should have a basic understanding of joint replacement equipment needs and implants. Responsibilities include coordination of care, telephone management, patient triage and patient education. Must possess strong assessment, communication and computer skills, be proactive and independent in decision-making and have good organizational skills.
