Social Worker MSW - Hospice

Schedule: Mon-Fri 8a-5p

This position will be traveling to see patients in Huntington Beach, Garden Grove, Orange and Santa Ana.

Job Description:  The Social Workers (MSW) evaluates and assesses patient needs for social service assistance, planning and coordination of clinical programs and acts as a liaison between the patient, hospital, Agency, Physician, Insurance Plans and community with in the St. Joseph Home Care Service Lines. Embraces and utilizes evidence based assessment and patient education tools. Understands and promotes the St. Joseph Home Health Social Service Program.

Required Qualifications:

• Master's Degree  Social Work.
• 1 year Social work experience.

Preferred Qualifications:

• upon hire: Social Worker License.
• Experience in home health care setting.

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
