Home Health Physical Therapist
DESCRIPTION OF POSITION:
- The Physical Therapist evaluates and treats all patients referred for physical therapy; prepares reports and communicates patient progress to appropriate individuals; participates in meetings concerning progress of patients; contributes to the development of the department.
SUPERVISION: Reports directly to the Home Health Clinical Supervisor
RESPONSIBILITIES AND DUTIES:
- Provides direct patient care, evaluates functional needs and outcomes, consults with other specialist as needed.
- Designs and implements a plan of care for the patient based on a thorough assessment of potential development and incorporates participation by significant others whenever it seems appropriate.
- Conducts therapy practice within defined standards of care.
- Directs the assistance of support staff in order to achieve the patient's functional goals.
- Communicates effectively with other professional and support staff in order to achieve positive patient outcomes.
- Documents evaluations, treatment goals and plans.
- Regularly updates documentation and maintains all required prescriptions and signatures.
- Provides direction and leadership to staff and acts as a resource to promote physical therapy within the community.

EDUCATION AND WORK EXPERIENCE
- BS degree in physical therapy from an accredited institution.
- Current licensure to practice physical therapy in the state of Louisiana.
- Must maintain required hours of in-service per year to meet requirements for CEU's as stipulated by the Louisiana State Board of Physical Therapy.
- CPR certification as outlined in the hospitals policies and procedures.
- Ability to communicate with patient, family, peers and medical personnel; Ability to exercise independent judgment.
- Requires emotional stability and maturity; Must be adaptable to change, be decisive, and demonstrate foresight; Exhibits organizational and leadership ability.
Physical Demands/Work Environment
- Work requires a variety of physical activities including standing and walking for long periods of time, bending, stooping, and lifting.
- The work environment may be physically confining.
- Visual acuity is required for a variety of specific activities.
- Must have normal hearing or utilize adaptive hearing devices if duties require listening with a stethoscope to discern breath sounds, heart sounds, and bowel sounds.
- Ability to do shift work as required.
