CTSI is pleased to offer a sign-on bonus for this position of $3800. The selected applicant shall receive one-half of the incentive in the first paycheck and the second half after the successful completion of six (6) consecutive months of employment provided the employee remains employed.  The Sign-on Incentive is not part of the base salary calculation and is considered taxable income.  The Sign-on Incentive is not included in retirement salary calculations.  A full copy of the incentive policy is available upon request.



INTRODUCTION:

The clinic RN provides quality safe nursing care to patients of Siletz Community Health Clinic while working within the scope of practice. This position assists providers and other team members in a wide variety of duties, as well as coordinating care to optimize patient outcomes. 



DUTIES:

  • All duties are expected to be performed in accordance with Siletz Community Health Clinic and CTSI policies and procedures, in addition to scope of practice, tribal, state and federal regulations.
  • Participates in assessing, planning and implementing in delivery of patient care in a primary care setting. Performs direct care activities by administering medications, vaccines, or other treatments as ordered then observes and documents patient response. Perform physical assessments, review histories, gather data, perform lab testing, then develop and implement nursing care plan. 
  • Provides ongoing planning, organizing, coordinating, monitoring, and evaluating of patient care in the clinic.
  • Consistently sets appropriate priorities in providing good patient care that is organized and time conscious.
  • Communicate with personnel from other clinic departments, tribal departments, and community health partners to coordinate resources, resolve problems, and coordinate services to provide high quality comprehensive patient care. 
  • Communicates with patients via phone, face to face, or email to deliver test results, discuss medication changes, assess patient complaints or concerns, communicate care plan, changes to care plan, provide education or instructions.
  • Create and maintain a high performace clinic environment characterized by positive role model, leadership and strong team ethic.Provides an environment that deomonstrates caring, compassion, and respect for all patients and families. 
  • Promotes and counsels patients regarding general health maintenance, preventative health, and encourages patient to be the leader in their own health care choices. 
  • Identifies medically and psychosocially complex patients and their families who are likely to benefit from care management. Coordinates referral to health navigator and ensures services delivered. Maintain and ensure confidentiality of patient’s conditions and records and other data on a consistent basis. Adhere to HIPAA guidelines at all times. 
  • Utilize motivational interviewing skills to encourage patients to set personal health goals. Assist patient to meet those goals via collaboration with other health professionals and/or referrals to appropriate treatment or education. 
  • Actively participates in Quality Assessment and Improvement activities. Serves as key team member that support evidence based best practices as outlined by Indian Health Service) and AAAHC benchmarks and standards.
  • Other duties as assigned by nursing supervisor, clinical services director, or medical director.


Requirements

REQUIREMENTS:

RN licensure in the State of Oregon. New Graduate RN’s with less than 2 years of experience will start at TL 124. After two years of satisfactory experience, position will transition to RN II at TL 125.2. RN’s with more than 2 years of experience will start at TL 125, with starting step negotiable depending on experience. 


Knowledge of: 

  • Medications, including potential side effects and interactions.
  • Knowledge of medical records, medical procedures, and terminology.


Ability to:

  • Recognize symptoms and provide recommendations within scope of practice.
