Job Summary

The Pharmacy Technician Liaison is responsible for accurately managing medication records, coordinating with healthcare providers, and providing patient education on their medications, with a specific focus on the needs of elderly patients, all while adhering to strict regulations and maintaining a high level of patient safety within the center’s pharmacy operations. 


Essential Job Functions

Duties include, but are not limited to:

· Medication Preparation and Dispensing:

· Receive prescription orders from PACE providers and outpatient specialty providers. 

· Process prescriptions electronically, ensuring all information is complete and accurately entered into the pharmacy system.

· Ensure accurate labeling and packaging of medications.

· Medication management:

· Maintain detailed patient medication records, including allergies, current medications, and refill schedules, updating participant address as needed, and ensure data privacy. 

· Perform medication reconciliation to ensure accurate medication lists. 

· Manage delivery of send to PACE medications and any undelivered or return to PACE medications.

· Receive and manage inventory of center administered medication

· Patient interaction:

· Receive and process Situation, Background, Assessment and Recommendations (SBAR) requests on the floor, via email or phone call to assist participant of their mediation needs.

· Provide patients with clear instructions regarding medication usage, side effects, and potential drug interactions. 

· Answer patient questions about their medications in a patient-friendly manner. 

· Identify potential medication concerns and communicating them to the physicians

· Coordination with healthcare team:

· Work closely with pharmacists, nurses, on call staff and other healthcare professionals.

· Collaborate with PACE clinicians to ensure timely medication delivery and address any medication-related issues. 

· Communicate with physicians regarding prescription changes or concerns. 

· Participate in care coordination meetings to discuss patient medication needs. 

· Coordinate with external pharmacies and suppliers as needed

· Inventory control of the Access to Medication Cabinet (ATM) and clinic medications:

· Monitor and maintain accurate inventory levels of medications and order supplies as needed.

· Perform regular inventory checks and replenish mediations as needed to prevent shortages. 

· Conduct routine inspections of medication storage areas.

· Attend and participate in staff meetings, in-services, projects, and committees assigned. 

· Adhere to and support the center’s practices, procedures, and policies including assigned break times and attendance.

· Act as pharmacy liaison to Operation’s Division leadership (Executive Directors, Center Managers, and Transportation Managers). 

· Accept assigned duties in a cooperative manner; and perform all other related duties as assigned.

· Be flexible in schedule of hours worked.

· Provide remote coverage for other centers as needed.

· May be required to use personal vehicle, if applicable. If using a personal vehicle, a valid California Driver’s License and proof of insurance is required. 


Knowledge, Skills and Abilities 

· Strong understanding of medication names, dosages, and interactions

· Excellent attention to detail and accuracy in data entry

· Strong organizational and Effective communication skills to interact with patients, caregivers, and healthcare providers

· Ability to work independently and as part of a team

· Attention to detail and accuracy in medication handling.

· Knowledge of geriatric pharmacy practices and common medication concerns for elderly patients 

· Proficient knowledge of computer skills. MS Office (Word, Excel, Access, PowerPoint, and Outlook). 

· Knowledge of general office procedures, equipment and filing systems.
