Job Description: Youth Services, Full Time

Job Summary

The Youth Case Manager assists in reducing youth barriers, communicating with youth care team (i.e. parents, counselors, schools, referral sources, etc.) and connecting to community resources for whole client care. Assists in handling appointments including roster management, chart creating, traveling to local sites to provide services, obtain intake information and aid youth counselors as needed.

ESSENTIAL FUNCTIONS

The Youth Case Manager is an integral role in carrying out the mission of The Counseling Center, Inc., by performing the following duties:

1. Assesses needs of client, assists in development of treatment plan, and gathers information upon admission to ensure communication with youth care team, medical providers and payee's and guardians are completed in timely manner.
2. Assists in obtaining supportive services to aid the client in their personal goals in recovery through the services of the agency or through referral to other agencies or programs.  Assist clients in accessing community resources and provides supportive services.
3. Provides group and individual services in an effort to improve coping and daily living skills in an environment that is conducive to recovery efforts.
4. Provides written documentation of client services in compliance with agency, OAC and compliance department standards. Youth Case Manager will assist in the development of treatment plans, weekly communication with referral sources as needed, and any other youth car team member. Initiates all correspondence and paperwork related to needs assessments and client service reports (i.e., letters, progress notes, rosters, trackers, mailing lists)
5. Attends weekly clinical staffing to assist with the discharge planning for aftercare, communication with youth care team, JBFS/insurance companies, and make referral.

SECONDARY FUNCTIONS

Secondary functions are duties, which are not exclusive of the position, can be performed by other positions; however, secondary duties are to be performed for the efficiency of The Counseling Center, Inc.

1. Performs other duties as assigned by the Program Coordinator/Director.

COMPETENCIES

1. Demonstrates Knowledge in Substance use disorders, community psychiatric supportive treatment, ASAM LOC, medical necessity as this relates to adolescents.
2. Demonstrates understanding of clinical practice and performance including understanding of the process of assessment, treatment, and confidentiality.
3. Ability in the area of effective communication, interpersonal skills, written communication
4. Ability to complete all documentation in accordance with agency policy.
5. Effective time management/organizational skills.

MINIMUM QUALIFICATIONS, INCLUDING TRAINING AND EXPERIENCE

1. High School Diploma or equivalent, Behavioral Science Degree preferred.
2. CDCA Required
3. Knowledge in Substance use disorders, community psychiatric supportive treatment, ASAM Level of Care and medical necessity.
4.  12-Step Recovery knowledge
5. Valid Ohio Driver’s License and ability to maintain insurability preferred.
6. Maintain CPR/First Aid treatment preferred.
7. Maintains applicable agency trainings, attends weekly supervision
