Job Opportunity: Skilled Hospice Nurse

We are seeking a dedicated Hospice Nurse who is committed to offering high-quality, patient-centered care in the comfort of their home. Position Overview:

As a Hospice Nurse, you will play a vital role in delivering personalized nursing care based on physician orders, utilizing your skills to promote comfort, dignity, and quality of life for our patients. You will work collaboratively with a multidisciplinary team to create care plans that support positive outcomes and meet the unique needs of each patient and family.

What You’ll Do:

  • Comprehensive Patient Care: Perform thorough assessments of patients and families to identify hospice needs. Provide physical assessments and history of current and past illnesses.
  • Skilled Nursing at Home: Deliver direct skilled nursing care based on individualized care plans and your professional expertise.
  • Collaborative Teamwork: Work alongside physicians, caregivers, and the interdisciplinary team to evaluate, update, and refine care plans to improve patient well-being.
  • Empathy and Support: Offer compassionate counseling to patients and families, empowering them with the knowledge and tools to manage care effectively at home.
  • Document and Report: Complete accurate clinical documentation in EMR systems, ensuring compliance with company standards and federal/state guidelines.
  • Flexibility in Care Management: Take charge of your caseload as a Case Manager when assigned, coordinating patient care and overseeing progress.

What We’re Looking For:

  • Education & Experience: Graduate of an accredited nursing program (BSN preferred), with at least one year of nursing experience. Hospice experience is highly preferred, as well as experience in caring for the elderly, chronically ill, and those with psychiatric needs.
  • Licensing & Certification: A current RN license in your state of practice, along with an active driver’s license and a reliable vehicle insured according to state requirements. BLS certification (American Heart Association or Red Cross) is also required.
